Nursing - Aged Care (Healthcare & Medical) ANACC CO-Ordinator East Melbourne We are looking to speak with experienced AN-ACC Co-Ordinators to join a large aged care home in the East of Melbourne.
As the ANACC Coordinator, you will provide site-specific support in relation to documentation and monitoring of resident care needs, while also maximizing funding and supporting compliance with the aged care industry.
Duties of the Role: Perform audits and provide support to the home during AN-ACC assessments.
Work with the clinical team to assist with reviewing & monitoring of AN-ACC assessments to maximize funding.
Assist with accurate completion of assessments & care plans.
Participate in regular resident reviews in consultation with the Clinical Manager and care staff.
Audit and evaluate assessments prior to the external AN-ACC assessors visit to ensure congruency and accuracy.
Actively liaise, mentor, and coach the appropriate employees in all matters relating to ANACC and documentation.
Key Selection Criteria: Excellent working knowledge of the processes related to AN-ACC.
Demonstrated ability to utilize AN-ACC to optimize funding/income.
Excellent verbal and written communication skills and ability to educate site-based staff.
Current/valid Police Clearance.
